DRAFT • Electronic records disclosure By checking the acceptance box and submitting the related booking or application, you affirmatively agree to the linked document versions. Acceptance is not inferred from browsing, silence, or a preselected checkbox. You need an internet connection and a current browser capable of displaying HTML and saving or printing pages. You can open every document before acceptance, print it, or save it using your browser. If you cannot access or retain the documents, do not accept electronically; request assistance through support. A paper-record request process and any actual disclosed cost must be established before live launch. We associate your acceptance with your authenticated account, the relevant transaction or application, the document version and content hash, and a server-generated timestamp. This is an evidence record; it does not itself guarantee enforceability or verify the identity of every person who uses an account. You may request copies of accepted documents and withdraw consent to future electronic contracting through support. Withdrawal does not undo completed transactions or existing lawful obligations. If electronic agreement is necessary to use the service, withdrawal may prevent new bookings; alternate arrangements must be addressed before live launch. Materially updated terms require renewed acceptance as applicable. Previously accepted versions remain available for the associated records. These disclosures require review for applicable federal and Indiana electronic transaction law before use with real customers.
